Mera Bharat Mahan 2- Hypoglycemia, starvation and Chandogya Upanishad

As per Chandogya Upanishad, liquid in the food makes urine, blood and Prana; solid food makes stool, muscles and manas; fatty foods make bone, bone marrow and karmendriyas (elimination, procreation, movement, grasping and speech).

Manas in Upanishad means Jnanindriyas (smell, taste, seeing, touch and hearing) along with mind, intellect, ego and chitta or pure intelligence (Fourfold mind or antahkarana chatushtaya).

Ahamkara is derived from the earth tanmatras; Chitta consciousness from jala tanmatra; buddhi discriminative mind from agni tanmatra; manas finite mind from vayu tanmatras and heart from akasha tanmatra.

The Upanishad says that a person can stay without solid food only for 16 days on liquids.

There is a saying in modern medicine that a person can stay without air for 3 minutes, without water for 3 days and without food for 3 weeks.

The symptoms of hypoglycemia will start once the food is not taken. The symptoms are mainly in the mental functions and sensory organs.

Once a person is in food starvation, even a small amount of food will take care of the mental symptoms.

Let us read the dialogue between Svetaketu with his father Udlaka in Chandogya Upanishad.

Chapter 6.7
6.7.1: षोडशकलः सोम्य पुरुषः पञ्चदशाहानि माशीः काममपः पिबापोमयः प्राणो नपिबतो विच्छेत्स्यत इति
ṣoḍaśakalaḥ somya puruṣaḥ pañcadaśāhāni māśīḥ kāmamapaḥ pibāpomayaḥ prāṇo napibato vicchetsyata iti||


O Somya, a person has sixteen parts, [and all your sixteen parts are intact]. Do not eat anything for fifteen days, but drink as much water as you like. Life is dependent on water. If you do not drink water, you will lose your life.

The mind consists of food, the prana consists of water and speech consists of fire.

6.7.2:   पञ्चदशाहानि नशाथ हैनमुपससाद किं ब्रवीमि भो इत्यृचः सोम्य यजूंषि सामानीति  होवाच  वै मा प्रतिभान्ति भो इति:
sa ha pañcadaśāhāni naśātha hainamupasasāda kiṃ bravīmi bho ityṛcaḥ somya yajūṃṣi sāmānīti sa hovāca na vai mā pratibhānti bho iti||

Śvetaketu did not eat anything for 15 days. After that he came to his father and said, ‘O Father, what shall I recite?’ His father said, ‘Recite the Ṛk, Yajuḥ, and Sāma mantras.’ Śvetaketu replied, ‘I can’t recall any of them, sir’.

6.7.3: तं होवाच यथा सोम्य महतोऽभ्या हितस्यैकोऽङ्गारः खद्योतमात्रः परिशिष्टः स्यात्तेन ततोऽपि  बहु दहेदेवंसोम्य ते षोडशानां कलानामेका कलातिशिष्टा स्यात्तयैतर्हिवेदान्नानुभवस्यशानाथ मे विज्ञास्यसीति
taṃ hovāca yathā somya mahato'bhyā hitasyaiko'ṅgāraḥ khadyotamātraḥ pariśiṣṭaḥ syāttena tato'pi na bahu dahedevaṃsomya te ṣoḍaśānāṃ kalānāmekā kalātiśiṣṭā syāttayaitarhi vedānnānubhavasyaśānātha me vijñāsyasīti ||

The father said to Śvetaketu: ‘O Somya, from a blazing fire, if there is but a small piece of ember left, the size of a firefly, it cannot bum anything bigger than that. Similarly, O Somya, because only one small part of your 16 parts remains, you cannot remember the Vedas. Eat something and then you will understand what I am saying’.

6.7.4:  हशाथ हैनमुपससाद तं  यत्किंच पप्रच्छ सर्वंह प्रतिपेदे  
sa haśātha hainamupasasāda taṃ ha yatkiṃca papraccha sarvaṃha pratipede ||

Śvetaketu ate something and then went to his father. Whatever his father asked him, he was able to follow.

6.7.5: तं होवाच यथा सोम्य महतोऽभ्याहितस्यैकमङ्गारं खद्योतमात्रं परिशिष्टं तं तृणैरुपसमाधाय प्राज्वलयेत्तेन ततोऽपि बहु दहेत् 
taṃ hovāca yathā somya mahato'bhyāhitasyaikamaṅgāraṃ khadyotamātraṃ pariśiṣṭaṃ taṃ tṛṇairupasamādhāya prājvalayettena tato'pi bahu dahet ||

The father said to him: ‘O Somya, from a blazing fire, if there is but a small piece of ember left, the size of a firefly, the fire can again blaze up when you add some grass. The fire, in fact, can then blaze up even more than it did before’.

6.7.6: एवं सोम्य ते षोडशानां कलानामेका कलातिशिष्टाभूत्सान्नेनोपसमाहिता प्राज्वाली तयैतर्हि वेदाननुभवस्यन्नमयंहि सोम्य मन आपोमयः प्राणस्तेजोमयी वागिति तद्धास्य विजज्ञाविति विजज्ञाविति 
evaṃ somya te ṣoḍaśānāṃ kalānāmekā kalātiśiṣṭābhūtsānnenopasamāhitā prājvālī tayaitarhi vedānanubhavasyannamayaṃhi somya mana āpomayaḥ prāṇastejomayī vāgiti taddhāsya vijajñāviti vijajñāviti ||

‘In the same way, O Somya, of your sixteen parts, only one remained. But that, when nourished by food, has revived, and by that you are now able to follow the Vedas. O Somya, this is why I said that the mind was nourished by food, prāṇa was nourished by water, and speech was nourished by fire.’ Śvetaketu now understood what his father was saying.

The 16 kalas are mentioned in the Prashna Upanishad 6.4.

 प्राणमसृजत प्राणाच्छ्रद्धां खं वायुर्ज्योतिरापः पृथिवीन्द्रियं मनः अन्नमन्नाद्वीर्यं तपो मन्त्राः कर्मलोका लोकेषु  नाम    
sa prāṇamasṛjata prāṇācchraddhāṃ khaṃ vāyurjyotirāpaḥ pṛthivīndriyaṃ manaḥ annamannādvīryaṃ tapo mantrāḥ karmalokā lokeṣu ca nāma ca ||

He created Prâna; from Prâna faith, âkâsa, air, fire, water, earth, senses, mind and food; and from food, strength, contemplation, mantrâs, karma and worlds; and in worlds name also.

1.     Prana or the life force
2.      Faith or Shraddha
3- 7. The Five Elements: Space, Air, Fire, Water and Earth.
8.     The Senses: The organs of knowledge and the organs of action.
9.     Mind, the Antahkarana or thought process
10. Food – that which sustains the gross body or the “Food-sheath”.
11. Veerya, or strength, mental and physical
12. Tapas or austerity
13. Mantra
14. Karma, the performance of actions
15. The World, the manifested world provides the field and the means to fulfill the actions undertaken.
16. The Naming of all the different items in creation.

#DrKKAnswers: There are three phases of fast: 1st phase is conversion of stored glycogen into energy and lasts a few days. 2nd phase is burning of fat into energy. In this phase acetone appears in the urine and this phase lasts as long as body fat is up to 10% in females and 7% in males. At this time there will be a strong urge to eat. If doe not eat at this time 3rd phase will start when proteins start breaking down leading to starvation effects which can be fatal due the kidney failure and heart irregularity.

Can a person die after his prolonged fast is broken?

Nine days fast by Ramdev, death of another swami in Uttarakhand after 115 days fast and now a threat by Anna Hazare to go into fast again has opened a new chapter for one to revise the knowledge as to how to deal with such a person especially one is breaking the fast.
The 35 years old swami who died was protesting the Uttarakhand government's refusal to ban mining along a stretch of Ganga near Rishikesh. He had been on a fast since February 19 2011. On April 27, officials shifted him to Haridwar district hospital as his condition deteriorated but he still refused to touch food. When he slipped into coma on May 2, he was rushed to HIMS and put on life support system.  The president of Haridwar Citizen Council, alleged the seer suffered a heart attack at Haridwar district hospital after being injected poison by hospital staff. Did he die of refeeding syndrome or the starvation effects is the question in the mind of the medical fraternity?
The basic aim of breaking any fast is to prevent occurrence of re-feeding syndrome, a metabolic complication that occurs when nutritional support is given to severely malnourished patients or to someone recovering from prolonged fast or hunger strike.
Any individual who has had negligible nutrient intake for 5 consecutive days is at risk. It occurs within four days of starting to feed. Patients who weigh less than 70 percent of ideal body weight or lose weight rapidly are at greatest risk for the syndrome.
During prolonged fasting the body conserve muscle and protein breakdown by switching to ketone bodies derived from fatty acids as the main energy source. The liver decreases its rate of gluconeogenesis thus conserving muscle and protein. Many intracellular minerals become severely depleted during this period, although serum levels may remain normal. Insulin secretion is suppressed and glucagon secretion is increased.
During re-feeding the metabolism shifts from a catabolic to an anabolic state. Insulin is released on carbohydrate intake resulting in increased glycogen, fat and protein synthesis. Insulin release triggers cellular uptake of potassium, phosphate, and magnesium which are already depleted and the stores rapidly become used up. When the serum concentrations of these electrolytes are reduced, serious complications, such as irregular heart beat can occur. Patients can also develop fluid and electrolyte imbalance, low phosphate levels, neurologic, lung, heart, neuromuscular, and blood related complications.  
Irregular heart rhythm is the most common cause of death with other significant risks including confusion, coma and convulsions and cardiac failure. The diagnostic feature is presence of low phosphate, potassium, magnesium, vitamin B1 levels  and fluid overload with edema feet. Low phosphate levels is the essential criteria for diagnosis.
If the refeeding syndrome occurs, doctors should immediately slow nutritional replenishment and correct low levels of phosphates, potassium and  magnesium while monitoring the cardiovascular system.
In patients at risk of re-feeding syndrome, nutritional support is initially delivered at a maximum of 10 kcal/kg/day and raised gradually to full needs within a week. In extreme cases this energy intake is limited to 5 kcal/kg/day.
Before and during the first 10 days of feeding, it is important to give 200-300 mg oral thiamine a day, 1-2 vitamin B complex tablets three times a day or intravenous vitamin B, and a balanced multivitamin and mineral supplement each day.
One should monitor and supplement oral, enteral, or intravenous potassium, phosphate and magnesium in such patients.
On average, patients should receive 2-4 mmol/kg/day potassium, 0.3-0.6 mmol/kg/day phosphate, and 0.2 mmol/kg/day intravenous or 0.4 mmol/kg/day oral magnesium.
Moderately to severely ill patients with marked edema or a serum phosphorous < 2 mg/dL should be hospitalized to intravenously correct electrolyte deficiencies.

How long Ramdev can take on fast

Rough formula of 3 is that one can not live for three minutes without air, three days without water and three weeks without food.

But this formula is for healthy muscular and fatty people. When you fast you can stay healthy for long if you have good muscles and have good fat content.

Minus points with Ramdev are that he is slim, his muscle mass is low, his body fat content is low and he is fasting in summer.

Plus Points with him are that he has n access to AC environment and he is a Yogi who are known to fast for long by reducing their body metabolic needs.

People who are inb extremes of ages or are suffering from a chronic ilment, prewgnnt ldies, diabetics should not fst without a doctors clernce.


Here are some facts
1. Fasting is willingly abstaining from some or all food, drink, or both, for a period of time.

2. An absolute fast is abstinence from all food and liquid for a defined period.

3. Partially restrictive fast is limited to particular foods or substance.

4. The fast may also be intermittent in nature

5. Fasting may refer to the metabolic status of a person who has not eaten overnight, and to the metabolic state achieved after complete digestion and absorption of a meal.

6. A person is assumed to be fasting after 8–12 hours.

7. Metabolic changes toward the fasting state begin after absorption of a meal (typically 3–5 hours after a meal); "post-absorptive state" is synonymous with this usage, in contrast to the "post-prandial" state of ongoing digestion.

8. A diagnostic fast refers to prolonged fasting (from 8–72 hours depending on age)

9. The longest known fast for a human is 132 days (without food).

10. Glucose is the body's primary fuel source and is essential for the brain's functioning. When denied glucose for more than 4–8 hours, the body turns to the liver for glycogen, a storage form of glucose, to be used for fuel.

11. A process called glycogenolysis converts glycogen into a usable form of fuel. At this point, the body also uses small amounts of protein to supplement this fuel. This fuel will last for up to 12 hours before the body needs to turn to glycogen stored in muscles, lasting for a few more days.

12. If glucose is still denied at this point, muscle wasting is prevented by temporarily switching to fat as the fuel source, meaning fat is converted into ketone through catabolism. Ketones, while not sugars, can be used by the brain as a fuel source as long as glucose is denied.

13. The body continues to use fat for as long as there is fat to consume.

14. The body will generally indicate to the faster when fat levels are running extremely low (less than 7% and 10% of body weight for males and females, respectively) with an increased urge for food. Fasts are usually broken long before this point.

15. If the fast is not broken, starvation begins to occur, as the body begins to use protein for fuel. Health complications associated with fast-induced starvation include electrolyte imbalances, thinning hair, lanugo, cardiac arrhythmia and renal failure. Death can occur if fasting is pursued to the point of complete starvation.

16. Hypovolemia refers to any condition in which the extracellular fluid volume is reduced.

17. It can be produced by salt and water loss (as with vomiting, diarrhea, diuretics, bleeding, or third space sequestration) or by water loss alone (ie, dehydration).

18. Salt and water loss comes primarily from the extracellular fluid whereas pure water loss (ie, dehydration) comes from the total body water, only about 40 percent of which is extracellular.

19. For dehydration to produce the same degree of extracellular volume depletion as salt and water loss, 2.5 times as much fluid would have to be lost.

20. Patients with dehydration are always hypernatremic whereas those with salt and water loss typically have a plasma sodium concentration that is normal or even reduced (due to free water replacement of part of the deficit).
