Any patient with acute left heart failure presenting as pulmonary edema, check the blood pressure if the blood pressure is normal or high unless proved otherwise, it is a case of diastolic heart failure (abnormality of the relaxation function of the heart) and carries good prognosis. If the blood pressure is low, it indicates low systolic or pumping function of heart and carries bad prognosis.

Just feel the hands and if they are warm, it means it is a shock with vasodilatation. The differential diagnosis is sepsis or anaphylaxis. If the hands are cold, it means vasoconstriction. Differential diagnosis is hypovolemia,  pulmonary embolism or acute heart attack.

  In a patient with cough of more than 3 weeks, one should suspect tuberculosis and do 3 samples of sputum examination and if positive, start with not less 3 anti-tubercular drugs.

Within 10 minutes of sudden cardiac death for the next 10 minutes one should do the chest compression with a speed of 10X10=100 per minute to save the life of the person. In Hindi, it can be remembered as “10 minute ke ander, das minute tak apni chhati peetne ke badle ulki chhati peeto”.
